Work, sleep, stress, diet, alcohol and smoking affect recovery and what is realistic to ask for each week.
Physical activity and availability
What the client has done before, the exercises they enjoy and dislike, and the days and times they are free. They help you set the right level and number of sessions a week.
Health
Diagnosed conditions, medication, injuries, surgery, current pain and pregnancy or postpartum. They show what to adapt and when to ask a doctor before starting.
Physical activity readiness
The client completes the official PAR-Q+ separately and you note on the form when it was done.

What is a client intake form?
It is the questionnaire for the first session. It gathers the client's details, goals, habits, training experience and health, so you can plan safe training that fits them.

It gives the address of the official PAR-Q+, eparmedx.com, where you can download it, and a line to note when the client completed it. The client fills in the PAR-Q+ separately.
