Frequently asked questions
What is a client intake form?
It is the questionnaire for the first session. It gathers the client's details, goals, habits, training experience and health, so you can plan safe training that fits them.
Does the form include the PAR-Q+?
It gives the address of the official PAR-Q+, eparmedx.com, where you can download it, and a line to note when the client completed it. The client fills in the PAR-Q+ separately.
Can I send the intake form to online clients?
You can send them the PDF to fill in on a computer or print. In MyFitnessClient you request the intake form in the app: the client fills it in on their phone and the answers go to their profile.
