Overview
The health questionnaire asks the client about their health and context, and they fill it in at the start. It provides important information for safer planning, including the PAR-Q (Physical Activity Readiness Questionnaire). You see the answers exactly as the client gave them.

By default, Measurements and Lab Values, Postural Analysis and Clinical Nutrition are turned off. Turn them on with Customize, in the Overview tab of the client page.
Request and review
You request the health questionnaire in the Overview tab of the client page; they get a notification and fill it in the app. When they finish, click View full record in the same tab to see the answers, section by section.
The app's questionnaire contains seven general readiness questions. It is not the complete official PAR-Q+ questionnaire and is not a medical assessment or diagnosis.
You can choose which sections are active and add custom sections, adapting the health questionnaire to your method.
